In order to best serve patients, Board Certified J. L Glashow M.D. is an out of network provider. Still, most insurance companies, when the patient has a policy that allows them to go out of their network, will reimburse patients for most of the costs.
Oxford, BCBS, and GHI will reimburse the patient for any surgery claims. Payment to Dr. Glashow for these surgery claims are due within ten days from the time the patient is reimbursed by their insurance company.
Our Surgical Coordinator, Elaine, will call your insurance carrier to obtain any necessary precerts or approvals. However a precert is never a guarantee of payment and benefits are determined at the time a claim is submitted. If your policy is not effective at the time of surgery because of a lapse in your coverage your surgery will not be covered. Please be sure to keep your insurance premiums current.

Dr Glashow’s Fees fall within most insurance carriers guidelines for the Usual & Customary range.
{Patients who are having multiple procedures with Dr. Glashow will be responsible for the PRIMARY procedure in full. Any additional procedures are payable at 50% of the surgical fee. The adjustments will be made in accordance with your INSURANCE GUIDELINES.}
